Dear Clients, Patients and Families of the Walden Help Centre:
As you may know, the Ministry of Health and Long Term Care announced its
intention to transfer district and local mental health and addictions
services in the Manitoulin and Sudbury District from the Northeast
Mental Health Centre (NEMHC) to the Sudbury Regional Hospital (HRSRH).
This transfer took place on June 1st, 2007.
Part of this transfer includes bringing together community mental health
and addictions services, from both the HRSRH and NEMHC, to one location
in downtown Sudbury at 127 Cedar St. Bringing these services together
represents an important opportunity to improve the quality and
efficiency of mental health & addictions care delivery for clients and
families in the Sudbury-Manitoulin District
Throughout the month of September, the HRSRH will begin moving services
from their current locations to the Cedar St. location. This includes
some of the staff currently located at the Walden Help Centre on 10th
Avenue in Lively, who were serving clients from other parts of the
district.
We are pleased to announce that a satellite site will
continue to operate in Walden. We recognize the importance of
locally available services to our clients, and have successfully
negotiated leased space in the Lively Medical Centre
on 6th Avenue. While the current Walden Help Centre will
close in mid-September, Brenda Punkari will be relocated to the Lively
Medical Centre to continue to serve clients and families from the Walden
area who have mental health counselling needs. These services can be
accessed through the existing phone number 692-5831 or through our
Central Intake and Referral Service at 523-4988.
Staff working with you from the Walden Help Centre will provide you with
specific information concerning the location and time of your next
visit.
The Lively Medical Centre is located at the corner of 6th Avenue and
Main St in Lively. Our counselling office will be located on the main
floor. Visits are by appointment only. Free parking is available.
A directional map showing our downtown Sudbury location is printed on
the back of this letter, for clients who are more conveniently served
downtown. Available parking is also noted on the back of this letter. We
encourage the use of the following parking lots in the area that provide
longer-term affordable parking: The Rainbow Centre (3 hours free), The
Market Square @ Cedar & Elgin St (2 hours free).

‘ Mental
Health’- Topic of Group Sessions at Sudbury Regional Hospital!
A
series of group sessions, designed to help people feel less alone in
coping with mental illness, will be held at the Sudbury Regional
Hospital in March.
The
SRH’s Mental Health Program will hold two-hour sessions, every Tuesday,
at 7pm on a variety of topics.
The
Families & Friends Education Group
will help those suffering from
mental illness, as well as their loved ones, understand how it affects
people and increase their knowledge about how to care for themselves.
Maureen McLelland, Administrative
Director of SRH’s Mental Health Program, says the service was piloted
last fall and was well received.
People interested may attend, any or
all of the sessions, which will be held at the
Community Mental Health Program on
the 1st Floor of the Laurentian Site.

Purpose
of the Centralized Intake & Referral Coordination Program
The
Centralized Intake & Referral Coordination Program
provides a central point of entry for individuals, family
members and service providers who are looking for support,
information, and services for adults with mental health
concerns. This service is also available to youth presenting
with eating disorders and symptoms of first episode psychosis.

Accessing the Centralized Intake & Referral Coordination
Program
A referral
can be made by phone, fax or walk-in. It can be completed by:

A family member

Pre-Treatment Groups:

recommended that clients attend all Pre-Treatment

The
Centralized Intake & Referral Coordination Program is
committed to ensuring a quick response to client needs by
providing an initial assessment. Admission to the recommended
Treatment Programs/Services varies according to individual
program availability.

Privacy and Confidentiality
One
concern that many people share is privacy of personal
information. The Centralized Intake & Referral
Coordination Program values and has the utmost respect
for the sensitive nature of personal health information. While
we are committed to keeping all client health information
confidential, there are limits to this confidentiality. Since
our program operates in collaboration with other mental health
services, discussion and consultation with other staff of the
Mental Health Program may occur in order to ensure appropriate
treatment recommendations and a high standard of care.
The
Centralized Intake & Referral Coordination Program must
abide by the Personal Health Information Act, 2004. Staff may
request a consultation with a client’s doctor, counselor, and
other H.R.S.R.H. partners and other services that are part of
the client’s circle of care. Unless the individual clearly tell
us not to, the provincial privacy law allows us to use and
disclose personal health information for reasons listed in the
Personal Health Information Protection Act, 2004 (PHIPA).
Information about this Act is available upon request.
If a
request that personal information not be shared, there are
situations/ communications that supersede this request. They
are:

We have reason to
believe that the client is a danger to himself/ herself
·
We have reason to
believe that the client is a danger to others
·
We have reason to
believe that a child under the age of 16 years is being abused
or is at risk of being abused/neglected
·
The client
discloses a sexually abusive act by a Regulated Health
Professional
·
The Program is in
receipt of a court-ordered request of information with a
subpoena
